Job Summary: The LTSS Health Coach is responsible for providing coaching and support to individuals eligible for both Medicare and Medicaid. This role involves developing health and wellness plans, educating patients and their families and coordinating services to ensure the best possible health outcomes. The ideal candidate will have a strong background in healthcare or social services, along with a passion for empowering individuals to take control of their health.
